Forms are required for meetings, activities, trips and travel. This page is to help guide you in determining what forms are needed, Forms can be found at the GSCCC Volunteer Forms and Documents page.
Annual Troop Forms
A set of the four basic forms is needed for EACH GIRL for ALL Girl Scout activities. A Health History form should also be collected from adults attending troop activities. These forms must be collected for each Girl Scout year (Oct-Sept) and carried by the troop or group leader for ALL activities. When split into vehicles, the driver must carry the forms for all individuals traveling within his/her/their vehicle.
Helpful Hint: Consider making multiple packets for travel to hand out to all drivers.
1. Annual Permission Form – Grants permission for girls to attend local meetings or events that are within 60 miles of the normal meeting place or no longer than 6 hours in length, participate in non-high-risk activities, and participate in product sales. ***Activities beyond this criteria require an approved Trip Form***
2. Girl/Adult Health History – Provides the girl’s pertinent health history, including medical conditions and allergies.
3. Permission to Release Girl Scout to Adult Other than a Parent/Guardian – Outlines to whom a girl may be released after meetings. Recommend they include leaders on this form, in the event a leader needs to pick up a girl after a council or group activity.
4. Medication Permission Form – Allows leaders to give prescribed or over-the-counter medication to girls as indicated. Recommend ensuring all items in troop First Aid kit are listed.

Additional Activity-Specific Forms
Activities and/or travel NOT covered in the annual permission form require additional permission from parents and the Council (as well as additional training). See the Trip Forms and Training Summary for more information. During the activity, carry these additional approved forms with you. Discard after 6 months or at the end of your Girl Scout year.

GSCCC Troop Travel Application Online Cognito form completed by the troop leader to seek GSCCC’s approval for activities / travel / overnights beyond the Annual Permission form, including High-Risk activities. Must be completed and approved BEFORE the start of any trip requiring approval.
Special Event/Trip Permission Form Completed by the parent to allow the girl to participate in a trip not covered in the Annual Permission form
High Risk Activity Permission Form Completed by the parent to allow the girl to participate in high risk activities (see form for activities listed)
Insurance
Additional insurance is required for activities/events where non-registered scouts will attend, or for all attendees of events in excess of three (3) nights. Please contact GSCCC Customer Care for more information. Apply for additional insurance well in advance of your trip/activity to ensure coverage before the trip begins.
